Introducing ‘Nourish’ at Ty Llandaff Care Home

From our initial care assessment prior to moving into the home and throughout a resident's time with us, we have personalised care plans for every resident which are recorded on Nourish. Through implementing this system, it empowers our care teams by using data to understand even better each of our residents. 

It provides insights to our team that allow for better decision-making and greater visibility for monitoring, analysing, and improving care practices to ensure that person-centred care is at the heart of everything we do.

Via handsets, that look like mobile phones, our care team is able to accurately record at the point of care, which in turn enables them to spend more quality time with our residents rather than having to manually update care plans.

Not only is the information recorded timely and accurately, but it also provides a further level of assurance for their loved ones of the care and support that they are receiving.
